Generally, most admissions paperwork will be handled by your physician’s office, by the hospital that makes your referral, or by our center. We also accept self-referred patients and patients whose families complete the admission process on their behalf.

If you (or a family member) are currently a patient in a hospital and you wish to be admitted to Park Manor Nursing and Rehabilitation Center, your best course of action is to inform your physician and/or hospital case worker as soon as possible of your preference.
